Better blood flow.
Better pumps.
But more muscle?

Tadalafil has a compelling mechanism. Following it from blood vessels to muscle cells reveals why the benefit depends on what was actually measured.

The direct answer: Tadalafil can influence vascular signaling, and human studies provide some encouraging metabolic and lean-mass findings. They do not establish a reliable hypertrophy benefit in healthy resistance-trained adults. The strongest case is a biological hypothesis worth testing, not a demonstrated muscle-building effect.[1][2][6][7]

The attraction is understandable: better circulation might deliver more nutrients, a fuller training pump feels like something is happening, and muscle-cell studies mention pathways familiar from growth research. Each step sounds reasonable. The difficult question is whether the whole chain has been tested in the same people, under the conditions that matter to a lifter.

Instead of asking whether tadalafil “works,” separate the immediate effect from the long-term claim. Improved erectile function, altered glucose handling and greater muscle size are three different outcomes. Evidence for one should make the next question interesting, not make its answer automatic.

CHAPTER 01

Tadalafil preserves a signal; it does not supply muscle

Nitric oxide, or NO, activates soluble guanylate cyclase, which generates cyclic GMP. That messenger helps vascular smooth muscle relax. PDE5 breaks down cGMP; tadalafil inhibits PDE5 and can prolong the signal. It is not an androgen-receptor agonist or a replacement for a training stimulus.[1][2][10]

FIGURE 01

Follow the NO–cGMP pathway

02 · cGMP productionDownstream signaling favors relaxation of vascular smooth muscle.
03 · Tadalafil inhibits PDE5Slows cGMP breakdown. It acts on signal removal, not on the supply of muscle protein.
04 · A vascular responseThe effect depends on the tissue, stimulus and physiological setting.
Simplified mechanism map. This describes vascular signaling, not the magnitude of blood-flow change in every tissue or a hypertrophy result.[1][2][10]

The distinction between smooth muscle and skeletal muscle matters. Relaxing the muscle in a vessel wall can change circulation. That does not mean the contractile fibers used for a squat are growing. The same word, muscle, is describing two tissues doing different jobs.

A mechanism also needs a limiting factor. If impaired microvascular function is restricting nutrient exchange, improving that function could matter. If another process is limiting adaptation, changing circulation may not move the final outcome. This is a reason to examine the study population, not a claim that circulation is unimportant.

CHAPTER 02

Blood arriving is not the same as glucose entering

Jansson and colleagues studied seven postmenopausal women with type 2 diabetes and ten healthy women. Each underwent placebo and single-dose oral tadalafil visits, four to six weeks apart; the research exposure was 20 mg, with measurements over four hours. Capillary-recruitment and forearm glucose-uptake estimates increased in the diabetes group, but not the healthy controls. Total forearm blood flow did not significantly change in either group.[2]

FIGURE 02

Three compartments, three questions

Bulk flowHow much blood passes through the forearm? No significant treatment change in this experiment.
Microvascular exchangeHow effectively can circulating glucose reach tissue? A favorable signal appeared in the diabetes group.
Tissue uptakeHow much glucose leaves the circulation into the forearm? This increased in that group, without demonstrating muscle growth.
Conceptual interpretation of Jansson et al. Capillary recruitment was estimated from glucose permeability-surface-area measurements, not a direct count of newly formed vessels.[2]

This is more interesting than a generic “more blood equals more nutrients” story. A tissue can change exchange behavior without a detectable increase in total flow. Conversely, seeing more visible veins would not tell you whether glucose transport into muscle cells improved. The visible sign and the metabolic measurement are not interchangeable.

It is also a population-specific result. A favorable response in insulin-resistant participants cannot be assigned unchanged to healthy athletes. That is not a dismissal of the study. It is the reason its control group is useful: the experiment itself tells us that biological context matters.

CHAPTER 03

An insulin-like signal is not an insulin-like clinical outcome

In 2013, Crescioli and colleagues exposed human fetal skeletal-muscle cells to tadalafil. They reported changes in insulin-associated signaling, including Akt and MAPK phosphorylation, plus selected metabolic effects. These were human-derived cells, not human volunteers. The experiment helps identify possible direct cellular actions; it does not measure a treated adult's insulin sensitivity or muscle gain.[3]

A later cell study examined IRS-1/Akt/mTOR signaling, GLUT4 trafficking and lipid handling. Its findings make nutrient storage a plausible research direction. Interestingly, sildenafil and a cGMP analog did not reproduce the reported GLUT4-trafficking and fatty-acid effects. That is a warning against assigning every finding to every PDE5 inhibitor.[4]

FIGURE 03

What the muscle-cell experiments actually establish

2026 · mouse C2C12 cellsGreater proliferation and survival; delayed terminal differentiationNo demonstrated human hypertrophy
Qualitative evidence map from abstracts. No unreported effect sizes, patient responses or equivalent tablet doses are inferred.[3][4][5]

The October 2026 experiment deserves particularly careful reading. Mouse C2C12 myoblasts received 1 micromolar tadalafil for up to seven days. The abstract reports more proliferation and survival, but fewer mature, myosin-heavy-chain-positive multinucleated myotubes. Early myogenic markers and reduced myostatin accompanied a less differentiated state.[5]

Those observations do not support a simple “myostatin down, muscle up” translation. Expanding a precursor population and completing its maturation are different stages. Whether retaining less differentiated cells later helps repair or limits mature tissue formation requires additional experiments. The favorable-sounding marker does not overrule the measured cellular phenotype.

Nor can a concentration in culture be converted directly into a tablet dose. A dish lacks the whole-body distribution, protein binding, metabolism and competing tissue effects of an adult. Mechanistic plausibility survives that limitation; a predictable human growth response does not follow from it.

CHAPTER 04

The human metabolic test did not give one uniform answer

MAKROTAD enrolled 23 people with well-controlled type 2 diabetes; 18 entered the full analysis set. It compared six weeks of oral tadalafil, 20 mg daily, with six weeks of placebo in a blinded crossover design, separated by an eight-week washout. Its primary clamp-based insulin-resistance outcome was neutral. Secondary HbA1c improved, while muscle pain, dyspepsia and headache were more frequent.[6]

FIGURE 04

Keep primary and secondary outcomes visible

SECONDARY · HbA1c−2.50 mmol/mol versus placebo
95% CI −4.20 to −0.78; P=.005
OTHER · endothelial functionFavorable signal; not evidence of hypertrophy.
BODY COMPOSITIONNo significant lean-body-mass percentage difference in the indexed main-text table.
MAKROTAD: within-person treatment comparison, not a comparison of unrelated groups. HbA1c values are the reported mean treatment difference and 95% confidence interval. Other rows are qualitative; no missing effect size is invented.[6]

The wider metabolic literature also resists a universal claim. Ho and colleagues analyzed 53 adults with obesity after three months of tadalafil or placebo: insulin resistance did not improve overall, despite favorable beta-cell and severe-obesity subgroup signals. A separate 28-day placebo-controlled trial in 18 men with obesity found no significant improvement in insulin secretion or sensitivity. These are distinct trials, not a head-to-head dose comparison.[11][12]

A glucose clamp asks how the body handles glucose under controlled insulin conditions. HbA1c reflects a different aspect of glycemic exposure. It is possible for those measurements to disagree without either being meaningless. The error would be to promote a positive secondary result into success on the primary question.

The small sample also matters in both directions. A nonsignificant result cannot exclude every modest benefit, but a small positive signal does not establish broad clinical effectiveness. Multiple secondary measurements offer opportunities to discover biology and opportunities for chance findings. Replication, prespecified outcomes and adequate statistical power help distinguish them.

For a lifter, the key unresolved step is larger still: even a convincing improvement in glucose regulation would not by itself quantify additional muscle gained during resistance training. To answer that, a study must measure the training outcome rather than merely propose a metabolic route to it.

CHAPTER 05

There is a human lean-mass finding. Read the comparator.

A 2017 trial randomized 43 nonobese men with mild erectile dysfunction and/or lower urinary tract symptoms to two tadalafil schedules for two months: 5 mg daily in 23 men, or 20 mg on demand in 20. DXA body composition was a primary outcome. The abstract reports increased abdominal lean mass in the daily group, returning to baseline two months after withdrawal.[7]

This should not be hidden simply because it complicates the story. Equally, the abstract does not supply a robust placebo-subtracted kilogram estimate. Both groups received active drug, and a significant change within one group does not establish a significant difference between groups. Without the full table, this review does not invent that comparison.

FIGURE 05

Lean mass is a clue, not a muscle-fiber measurement

WHAT WAS TESTEDTwo active schedules

WHAT THE CLAIM REQUIRESLasting hypertrophy

A direct muscle-size outcome, controlled training and a credible between-group comparison.

Regional DXA lean mass ≠ direct measurement of muscle-fiber growth.
Study design from Aversa et al.; measurement interpretation is an editorial framework. No assumption is made that the reported change was entirely water or entirely contractile tissue.[7]

DXA partitions tissue rather than counting new contractile proteins. A regional lean-tissue change therefore needs context, including hydration, tissue composition and the region analyzed. Dismissing the whole result as water would be as unsupported as calling all of it new muscle. The honest interpretation leaves the tissue-level explanation open.

The withdrawal observation adds another question: durability. Returning to baseline does not identify what originally changed or prove that no useful effect occurred. It does mean the finding cannot simply be described as a permanent gain. Magnitude, composition, function and persistence should all remain attached to a physique claim.

CHAPTER 06

A pump, a performance result and a growth result need separate tests

Acute athlete experiments provide a useful reality check. A double-blind crossover study in 14 healthy male athletes found no improvement in maximal oxygen uptake or aerobic/anaerobic thresholds under normal oxygen conditions. That does not settle every exercise question, but it prevents turning vasodilation into an automatic endurance benefit.[8]

Another crossover study tested 12 trained participants using a 30-second Wingate cycling test after tadalafil or placebo. Mean and peak power did not improve. Time to peak power and one recovery lactate measurement changed. Those selected findings should be retained without relabeling the overall experiment a major power enhancement.[9]

More recently, a 2026 exploratory human multi-omics study examined muscle and blood samples. Eight completers received tadalafil and eight sildenafil, each with a drug/placebo crossover; active treatment lasted four weeks. The reported molecular analyses compared before versus after active treatment. Tadalafil produced no significantly differentially expressed muscle genes and no enriched biological processes among the altered proteins, unlike the more coherent sildenafil signal. This was small and exploratory, not a hypertrophy trial or proof that one drug builds more muscle.[10]

FIGURE 06

What would close the hypertrophy evidence gap?

  1. A relevant populationResistance-trained adults, with baseline health and other drug exposure documented.
  2. A fair comparisonRandomized placebo control, comparable training and nutrition, and documented adherence.
  3. Direct outcomesPrespecified muscle-size measurements alongside strength; lean mass reported separately.
  4. Enough time and safety dataFollow adaptation beyond an acute pump, account for withdrawals and assess persistence.
Original study-design framework. These are proposed requirements for testing the claim, not results from an existing tadalafil trial.

Reports of fuller pumps are observations about experience. They can generate a hypothesis, but expectation, workout selection, carbohydrate intake and other substances complicate attribution. A before-and-after photograph does not isolate the drug. A more satisfying session might affect training behavior, yet that proposed indirect benefit also needs testing.

It would be equally premature to argue that tadalafil can never influence adaptation. Current findings leave room for population-specific or indirect effects. The useful conclusion is narrower: a person cannot use this evidence to predict how much extra muscle tadalafil will produce, or whether a noticeable pump identifies someone who will respond.

CHAPTER 07

The vascular mechanism also explains important interactions

The current U.S. CIALIS label covers erectile dysfunction and benign prostatic hyperplasia, not muscle building. Familiarity should not obscure contraindications: organic nitrates and guanylate-cyclase stimulators such as riociguat must not be combined with tadalafil. Serious hypersensitivity is another contraindication.[1]

FIGURE 07

Risks that change the interpretation

Blood-pressure interactionsAlpha-blockers, antihypertensives and substantial alcohol can add to hypotension.
Drug exposureCYP3A4 inhibitors and kidney or liver impairment can affect exposure and prescribing decisions.
Common adverse effectsHeadache, indigestion, back pain and muscle pain can matter for training.
Urgent symptomsAn erection lasting over four hours needs emergency care; sudden vision or hearing loss requires prompt medical attention.
Selected current label warnings, checked October 29, 2026. This is not a complete prescribing guide.[1]

Chest pain also needs urgent assessment, with recent tadalafil use disclosed because of the nitrate interaction. A vascular drug is not evidence-based protection against the cardiovascular effects of anabolic steroid use. Adding one plausible mechanism does not cancel a different exposure's harms.

That last distinction matters when interpreting combinations. A study of tadalafil alone cannot establish the net outcome of tadalafil plus several other drugs. Benefits, adverse effects and interactions do not necessarily add linearly. Research about one pathway should not be presented as a protective protocol.

A credible mechanism. An unresolved muscle-building claim.

Tadalafil is worth understanding because vascular function, nutrient exchange and muscle-cell signaling intersect. Human evidence includes an intriguing regional lean-mass finding, favorable metabolic signals and important null results. Together they justify further research. They do not yet establish predictable, lasting hypertrophy in healthy lifters.
